Keto and intermittent fasting: how to combine them without the keto flu

Keto and intermittent fasting get paired because they push your body toward the same fuel. Cut carbs to a few dozen grams a day, stretch the gap between dinner and your first meal, and your liver makes more ketones from fat. Plenty of people start both on the same Monday and feel terrible by Wednesday.

Research on the pair is thin: a few small trials, most of them short or in people with a specific disease. The steps below follow the order that works in practice, from keto on its own to the signs that the plan has become too strict for you.

1. Know what keto and intermittent fasting each change

Intermittent fasting limits when you eat. Keto limits what you eat, usually to about 20 to 50 g of carbohydrate a day, with fat making up most of your calories. For scale, one medium apple holds about 25 g.

In healthy adults, total blood ketones normally drift between about 0.1 and 0.25 mmol/L over the day and rise to about 1 mmol/L after 24 hours without food. Nutritional ketosis is usually defined as beta-hydroxybutyrate, the main ketone, at 0.5 mmol/L or more, and a well-formulated keto diet typically keeps it between about 0.5 and 3 mmol/L. Diabetic ketoacidosis, the dangerous state, can reach 20 mmol/L.

The routes differ. A fast runs down glycogen, the glucose stored in your liver, and ketones climb as it does. Keto keeps carbs so low that the liver makes ketones even on days with three meals.

2. Ask whether keto gets you into ketosis faster on a fast

It is plausible. On keto you likely start each fast with less glycogen and with ketones already raised. One small study points the same way: 11 healthy adults ate a mixed diet for five days, then a single day of either normal or very high-fat food, then fasted for a day. After the high-fat day, acetone on their breath, a marker of ketone production, built up more during the fast.

That is one high-fat day in 11 people, measured on breath, so treat it as a hint. The same study found that people with a lower body mass index and a higher resting energy expenditure built up more ketones whatever they had eaten, and activity shifts the timing too. Once you are adapted to keto, you are usually in mild ketosis around the clock, and the fast adds hours on top. Our timeline of when ketosis starts during a fast covers the mixed-diet version hour by hour.

FastTrack's timer shows the fasting phases up to ketosis as a rough guide based on the hours since your last meal. It does not measure ketones; a blood ketone meter is the way to know your number.

3. Start keto first, then add the eating window

Clinics traditionally began a ketogenic diet with a fast. A 2025 review of symptoms during the switch to keto lists dropping that fasted start among the approaches reported to make the first days easier. So keep long fasts out of your first week without carbs:

  1. Weeks 1 and 2: keto at your usual meal times, with about 12 hours overnight, say 7 pm to 7 am.
  2. Week 3: breakfast at 9 am, for a 14-hour fast.
  3. Week 4: if late mornings feel easy, first meal at 11 am for 16:8.

4. Get through the keto flu without quitting

Keto flu is the name people give to feeling run-down in the first days of cutting carbs. The 2025 review found reports across studies of keto flu itself, nausea, vomiting and lower appetite, along with problems such as low blood sugar and a higher risk of kidney stones. It called the symptoms transient and noted that few studies have examined them directly, so most of what helps comes from practice:

  • Keep the fast short that week. Twelve hours overnight is plenty.
  • Eat full meals. Keto food is filling, and with a window on top it is easy to eat far less than you need.
  • Add vegetables for fiber. Bread, rice, beans and most fruit leave your plate, and much of your fiber goes with them. A cup of broccoli or two cups of greens per meal, plus a tablespoon of chia seeds, makes up some of it.
  • Salt your food and drink water. Step 5 has amounts.

The review also lists MCT oil and ketone salts among reported aids, with little trial evidence behind them. MCT oil adds about 100 calories a tablespoon, so keep it in your eating window.

5. Plan electrolytes for keto and intermittent fasting together

Both halves of the plan can cut your salt. If sandwiches and canned soup gave way to keto meals cooked from scratch, you may eat less sodium than you think, and the fast removes a meal. The 2025 review saw a physiological case for electrolytes during the switch and a lack of clinical studies showing that they help, so this part rests on reasoning and experience:

  • Sodium: salt your food to taste, or sip 1/8 to 1/4 teaspoon of salt in 16 ounces of water during the fast.
  • Potassium: half an avocado, a cup of cooked spinach, mushrooms or a salmon fillet. Skip potassium powders and salt substitutes unless your doctor agrees, especially with kidney disease or pills such as lisinopril or losartan.
  • Magnesium: an ounce of pumpkin seeds or almonds.

Our guide to electrolytes while fasting covers amounts by fast length. With high blood pressure, heart failure, kidney disease or a water pill, ask your doctor before adding salt.

6. Build keto meals that fit your eating window

On a noon to 8 pm window, two meals are the usual shape. Build each around 25 to 40 g of protein, since plans heavy on butter and cream leave little room for it:

  • Noon: a three-egg omelet with spinach, mushrooms, scallions and an ounce of cheddar, half an avocado on the side, about 25 g of protein and 5 g of net carbs.
  • 7 pm: 6 ounces of baked salmon, a cup of roasted broccoli with olive oil and a side salad, about 40 g of protein and under 10 g of net carbs.

Eating out, a bunless burger with salad or a burrito bowl without rice or beans fits both plans.

Bulletproof coffee is where the two collide. A cup with a tablespoon each of butter and MCT oil keeps most people in ketosis, and at 200 to 250 calories it still ends the fast; our page on whether bulletproof coffee breaks a fast has the numbers. If you drink it, count it as your first meal.

Carbs hide in sauces and so-called keto bars. FastTrack's meal scan estimates calories and macros, carbs included, from a photo of your plate.

7. Spot the signs the combination has become too restrictive

Two sets of rules at once is a lot to live with. One randomized trial put 68 young adults with overweight or obesity in a control group or on a time-restricted keto diet, with or without exercise, for four weeks and without counting calories. Weight fell by 5 to 6% on average in the keto groups, and sleep quality got worse in all three, most in those who lost the most weight. The authors call the sleep effect temporary, though four weeks was too short to show when it settles, and an average says little about what you will see.

A 2026 meta-analysis of 62 randomized trials found that keto lowered triglycerides by about 20 mg/dL and raised LDL cholesterol by about 8.5 mg/dL on average. Long-term heart outcomes are unknown and individual responses differ, so ask for a lipid panel before you start and again after about three months.

What you noticeWhat to change
Sleeping worse for two weeks or moreWiden the window to 10 or 12 hours
LDL well above your baseline on the follow-up testTake the result to your doctor before carrying on
Thinking about food all day, or bingeing when the window opensDrop the window, and talk to a professional if it continues
Strength falling, or weight dropping fast without tryingAdd a meal or widen the window
Missed or irregular periodsStop the window and see your doctor

8. Know who should skip it and when to stop

Skip the combination, or do it only with your doctor's plan, if you:

  • Have type 1 diabetes, where low insulin and high ketones set the stage for ketoacidosis.
  • Have type 2 diabetes treated with insulin, a sulfonylurea such as glipizide, or an SGLT2 inhibitor such as empagliflozin. Insulin and sulfonylureas can push blood sugar too low, and SGLT2 inhibitors carry a risk of ketoacidosis even with normal blood sugar, which very low carb intake adds to.
  • Are pregnant or breastfeeding, or under 18.
  • Have a history of an eating disorder.
  • Have kidney disease.

Stop the fast, eat something with carbohydrate and get medical help if you notice dizziness or fainting, palpitations, confusion, vomiting, or fast, deep breathing with stomach pain. With diabetes, check your blood sugar and ketones and call your care team.

Is keto with intermittent fasting worth it?

For healthy adults who like keto food, a 14:10 or 16:8 window can sit on top of it, and ketosis may come earlier in each fast. The combination has only small, short trials behind it, and the clearest found weight loss alongside worse sleep. Start keto first, salt your food, keep protein in both meals and check your LDL. If sleep or training slips, loosen the window before you give up on either plan.

Frequently asked questions

Does intermittent fasting on keto get you into ketosis faster?

It is plausible, since your carbohydrate intake and glycogen stores are already low when the fast begins. In one small study, a single very high-fat day before a fast made breath acetone, a marker of ketone production, build up more during the fast. Body size and activity change the timing, so it varies between people.

What is the best eating window for keto?

No window has been shown to be best. Many people settle on 16:8, such as noon to 8 pm, after two or three weeks of keto alone, and 14:10 is a gentler start. Pick the window that keeps dinner with your family or friends.

Can I drink bulletproof coffee while fasting on keto?

It will usually keep you in ketosis, and with 200 to 250 calories of fat a cup it ends the fast. Count it as your first meal and start your eating window when you drink it.

How long does the keto flu last?

Reviews describe it as transient, though few studies have tracked it closely, so there is no reliable number of days. Keep your fasts short while it lasts and salt your food. If symptoms are severe or include vomiting, confusion, fainting or palpitations, stop and see a doctor.

Will keto and intermittent fasting raise my cholesterol?

In a meta-analysis of 62 trials, keto diets raised LDL cholesterol by about 8.5 mg/dL on average while lowering triglycerides. Individual responses vary, so get a lipid panel before you start and again after about three months.

Is it safe to combine keto and fasting with type 2 diabetes?

If you take insulin, a sulfonylurea or an SGLT2 inhibitor, only with your doctor's plan, because the combination can cause low blood sugar or ketoacidosis and doses often need to change first. On other treatments, talk to your doctor before you start.
