Heavy cream is mostly fat with very little sugar or protein, so it barely moves insulin and will not break ketosis. But it is calorie-dense, around 50 calories per tablespoon, so it does break a fast defined by calories and it shuts down autophagy. People doing a fat-loss or keto-style fast sometimes allow a splash; people fasting for autophagy should not.
If you only care about staying in ketosis, a small splash of heavy cream is tolerable. If you care about calorie restriction or autophagy, it breaks the fast. And watch the amount, since a tablespoon or two in coffee adds up quickly.
